The purpose of this project to obtain services for the development of a strategic framework and measurement development plan for the Centers for Medicare and Medicaid Services (CMS). This includes identifying and explicating measures that currently exist or are in development to assess population health strategies, their potential application to CMS payment policies and CMS-funded models, and gaps in measuring population health strategies. A second phase of this project is to adopt and/or develop a minimal set of measures to track the processes and outcomes that directly relate to the population strategies of health care providers. These measures can be used to assess local or organizational performance, inform action, and/or monitor progress towards improvement.
